Sports Academy Registration Form and Season Checklist
Design a registration intake that is clear for families and easy for staff to review, without confusing an operations form with a legal agreement.
Start with the job the form needs to do
A registration form should help the academy create an accurate student record and contact the responsible adult. It does not need to become a long survey.
Keep the intake readable on a phone. Group related questions, use plain labels, and explain what a family should do when a detail is not yet known. Avoid asking for extensive health, identity or family information by default; follow the school’s approved privacy process for any sensitive information it truly needs.
Collect contacts and prepare an intake draft
The core record usually needs the student’s name, the guardian’s name, and a reliable way for the academy to reach that guardian. If the student is an adult, collect the appropriate contact details directly. Confirm spelling and contact preference at intake, and explain which notices the academy may send through each channel.
Where more than one guardian should receive updates, make the intended recipients explicit and verify their contact details. Do not assume that an address entered for one family member should automatically be used for another, or that a contact has permission to act for every student in the household.

Record placement and schedule needs
Ask which sport or program the student is joining and the group or age band the family is requesting, if the academy uses those distinctions. Staff should confirm the actual placement and schedule rather than treating a family’s preference as a final assignment. Record the assigned group in the student record so coaches receive a usable roster.
If the academy needs an emergency contact or a specific participation detail, explain why it is requested and handle it according to the school’s process. Do not add broad open-ended questions that invite families to share information the staff cannot safely manage.
Explain fees and payment recording clearly
Tell families how the academy communicates its fees, periods and due dates. Use the current fee schedule and currency, and give parents a contact for questions or corrections. Keep operational intake separate from agreements

Review the form before inviting families
Test the form with a staff member on a phone, then ask someone unfamiliar with the process to complete it using sample details. Watch where they hesitate. Fix ambiguous terms, remove repeated questions, and check that the confirmation tells them what happens next without promising a specific start date or team placement before it has been reviewed.
Decide who reviews new submissions, how corrections are handled, and how the office confirms the assigned group and schedule. Provide a contact for incomplete or duplicate submissions. A form is only useful when there is a clear process behind it.

A student import CSV is a data-transfer template, not a family-facing registration form. It is meant to help staff move reviewed records into a system using the expected column names and format. Ask families for information through the academy’s chosen intake process, then have an authorized staff member check it before importing.
Before an import, use one student per row, preserve the source file, check group names for consistency, and resolve missing or duplicate records. Import a small sample first when possible, compare the result with the source, and retain a secure copy according to the academy’s retention process.
After intake, keep the student record connected

A returning player usually needs a record check rather than a new intake. Confirm the student and guardian contacts, intended group and season, current schedule and start date. Ask the family only for details that changed or are missing; then review fee details against the academy’s current schedule.
